Application information for NEW passports and renewals for expired passports (issued before this date in 2010).
Standard processing time is 4 to 6 weeks.
Expedited processing reduces delivery time to 2 to 3 weeks for an additional fee of $60.00, plus (optional) overnight delivery fees.
WHAT YOU NEED FOR YOUR APPOINTMENT:
Your original (or certified copy) state-issued birth certificate– document MUST have a raised or embossed seal and both parents’ full names
Your driver’s license, state-issued ID card, or another form of photo identification
COMPLETED online Passport Application Form DS-11, or you can print it out to complete by hand or pick up a form from the library. Black ink required.
One passport photo or we can take your photo here at the library ($15.00)
A CHECK or MONEY ORDER Made payable to the U.S. Department of State
Note: Payment MUST be made by check or money order. If payment is made by someone other than the applicant, that person must be present and provide a photo ID.
Passport Book Fee
Adult $130.00 (valid for 10 years)
Child under 16 $100.00 (valid for 5 years) (minors must always complete
Passport Card Fee (Cards are limited for use of land or sea border travel to Canada, Mexico, Bermuda, and part of the Caribbean. They are NOT valid for any international air travel.)
Adult $30.00 (valid for 10 years)
Child under 16 $15.00 (valid for 5 years)
US Department of State mandated fee of $35.00. (This fee can be paid with cash, credit/debit, or check payable to Hamlin Community Library.) If we are taking your photo, these fees can be combined into one payment.
You must apply in person using Form DS-11 if at least one of the following is true:
You are applying for your first U.S. passport
You are under age 16
Your previous U.S. passport was issued when you were under age 16
Your previous U.S. passport was lost, stolen, or damaged
Your previous U.S. passport was issued more than 15 years ago
